We will need you to complete the 2025-2026 Identity and Statement of Educational Purpose Form first. This form must be completed in the presence of a Montgomery County Community College Enrollment Services representative. Please bring your original government issued identification such as a driver's license, state ID, or passport when you arrive to complete the form. The form may also be completed in the presence of a notary public, by which the original signed and notarized form must be mailed to us along with a copy of your government ID.
